The Arc’s HRC will review financial rights restriction cases monthly, on the 3rd Wednesday of every month at 4p.  The committee will meet at 215 University Dr in Fargo at the Arc of Cass County office.  In-person presentations are not required but can be accommodated upon request.

 

Please download, fill out, save then send completed forms  to hrc@arccassnd.com. An email will be sent to confirm receipt along with the expected date of review.

 

If in-person presentations are requested, please include your request in the email containing the completed review form. When submitting the review form, please ensure that it is filled out completely, and remember to use initials throughout the form avoiding the usage of the service recipient’s name.  Please note that individual service provider forms will be accepted if all necessary information is provided.
